The Board is prohibited by law from taking action on matters not on the agenda, but may ask questions to clarify the speaker's comment. The Board limits testimony on matters not on the agenda to 3 minutes per person and not more than 10 minutes for a particular subject at the discretion of the Chair of the Board.

Individuals wishing to address the Board under Public Expression are welcome to do so throughout the meeting. HEALTH AND HUMAN SERVICES AGENCY

(g)        Approval of Agreement with Sonoma County in the Amount of $99,513.64 for the Provision of Emergency Medical Services (EMS) Administration in Mendocino County for Fiscal Years 2013/14, 2014/15, and 2015/16

Recommended Action/Motion: Approve and authorize the Board Chair to sign the Agreement with Sonoma County for the Provision of Emergency Medical Services (EMS) Administration in Mendocino County for Fiscal Years 2013/14, 2014/15, and 2015/16, and authorize the HHSA Director to sign any future amendments to the agreement that do not affect the annual maximum amount.

(h)        Approval of Amendment to Purchasing Agent Agreement 12-24 with Creekside Convalescent Hospital (Santa Rosa, CA) Increasing Agreement Amount by $42,565 for a New Total Agreement of $92,565 to Provide Residential Treatment Program to Mental Health Clients in Fiscal Year 2012/13

Recommended Action/Motion: Approve and authorize the Board Chair to sign amendment to purchasing agent agreement number 12-24 increasing agreement amount by $42,565 for a new total agreement amount of $92,565 to provide residential treatment program to mental health clients in Fiscal Year 2012/2013; and authorize the HHSA Director to sign any future amendments to the agreement that do not increase the annual maximum amount.

(i)          Approval of Appropriation Transfer of Funds, Within Budget Unit 0478 Disaster Preparedness, from Line Item 86-2239 Departmental Special Expense to Line Item 86-4370 Fixed Assets, Totaling $28,000

Recommended Action/Motion: Approve and authorize the Board chair to sign Appropriation Transfer of funds within Budget Unit 0478, Disaster Preparedness, from line item 86-2239 Departmental Special Expense to line item 86-4370 Fixed Assets in the amount of $28,000.

TRANSPORTATION

(j)          Adoption of Resolution Approving Award of Department of Transportation Agreement No. 120055, Professional Services Agreement with Associated Right of Way Services, Inc. for  Right of Way Acquisition Services for the Reconstruction and  Widening Project for East Side Potter Valley Road, CR 240, M.P. 2.40 to M.P. 6.42 (Potter Valley Area)

Recommended Action/Motion: By Resolution, approve DOT Agreement No. 120055, authorize/direct Board Chair to execute said Agreement on behalf of Mendocino County and authorize the Director of Transportation or his designated representative to sign any and all amendments to this Agreement that will not result in an increased cost beyond the specified amount.

TREASURER-TAX COLLECTOR

(k)        Authorization of the Discharge from Accountability, the Accounts with Debt Over 10 Years Old or Determined Non-Collectible, Comprised of Fines, Penalties, Fees, and Assessments, as the Cost of Recovery Exceeds that of Collection

Recommended Action/Motion: Authorize the discharge of accountability, the submitted listing of delinquent fines, penalties, fees and assessments, and write off the debt.
