The Child Health and Disability Prevention Program (CHDP) delivers health check-ups to infants, children and youth and provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services Children eligible for CHDP services are both:
Health check-ups are needed on a regular basis throughout the child's life. Babies and toddlers need health check-ups often, every few months; they can get their shots during these exams. School age children need a check-up to enter kindergarten and every couple years; these exams may find health problems that could prevent a child from doing well in school. Teenagers and young adults need health check-ups too. They may have questions about their health that they can ask a health care provider. Health exams are often required when children play sports or go to summer camp and CHDP exams can be given every year to meet these requirements. Getting complete health check-ups for children is important because these exams may prevent serious health conditions and disabilities by finding medical, dental and/or mental health problems early. Referrals are made for any health or dental conditions identified during the exam. A CHDP program nurse provides follow-up and assistance to families for diagnosis and treatment of problems that are found.
