IMPORTANT NOTICE ABOUT TIME SENSITIVE NOTIFICATION OF HEALTH PLAN CHANGESWhat is a Qualifying Event? Answer: A qualifying
event is a special circumstance that allows you to make changes to your health
care plan without waiting until the next plan year in January. Examples could be the birth of a
child, loss of other health care benefits, enrolling in other health care
benefits, divorce, marriage, legal adoption, or the death of a covered family
member. Are There Time Limits? Answer: Yes, per the Mendocino County Summary Plan Description, pages 10-11, you only have a period of 31 days after the date of the qualifying event to complete the appropriate forms and provide verification to your Health Benefits Specialist to have the update to your benefits completed. The Summary Plan Description is available. What if I Miss The Deadline? Answer: If you miss the 31 day deadline to make changes, you will have to wait until Open Enrollment to make the changes. That means you could end up paying for benefits you no longer need or having to pay for your full medical expenses directly out of your pocket until the beginning of the next plan year in January. notify DeAnna Graham, Health Benefits Specialist at (707)463-4445 or email her. She will provide you with the appropriate forms for your update and answer
any questions you may have regarding your changes.
